For hospital case managers and discharge planners

You're responsible for a safe discharge, not for being a senior living directory. We take the family's search off your plate and report back to you — at no cost to the hospital, the family, or you.

  • No PHI. We work from a discharge date, care level, payer situation and a family contact — never the chart.
  • Same-day response for active discharges. Every option checked against the DSS licensing record before the family sees it.
  • Skilled nursing, home health and hospice too — we help the family navigate those at no charge to anyone.
What we will never do: pay for a referral, offer anything in return for one, or ask you to steer a family. Families choose.

    We schedule the visits and come along if they'd like. The decision belongs to the person and their family, with their doctors.

The questions people ask first

Is it really free? What's the catch?

It's free to the family because communities pay us when a family we introduced moves in. That's the catch, and it's the same arrangement every senior living advisor has — we're just the ones who say it out loud and put it in writing.

Do you only show places that pay you?

No. If a community outside our network is a better fit, we'll say so. On every option we present, we'll say whether it's one that pays Waypost Care.

I'm a case manager. Do you pay for referrals, or expect anything back?

No, and we never will. Nothing changes hands between Waypost Care and a hospital, a case manager, or a provider who sends a family our way.

How fast can this happen?

If there's a discharge date, tell us and we'll work to it. A first set of options in writing typically follows within a couple of days; tours can usually be arranged for the following day.

Will you pressure the family to decide?

No. The decision belongs to the person and their family, with their doctors. Our job ends when they have what they need to make it.
